Request Form for an Interpreter
  1. Fill out this form three weeks in advance for all requests for interpreters to attend IEPs or other parent meetings/ events. 
  2. Send request form to Kendra Seitz, ESL Coordinator (via interschool mail to Upton, email or fax).
  3. Stipend form for an interpreter: Print/ bring to meeting/ event. Fill out with interpreter and send completed form to Margie Martin at the admin building.
